How to contact us...
My child is appearing in a television programme and I've been told I need an Entertainment Licence - how do I get one ? Application forms are available from Attendance and Pupil Support or the Kirklees Information Points. Alternatively, download and print the application form (PDF 100Kb) When you return the form, you must supply 2 passport sized photographs, your child's birth certificate and a schedule of work. I would like to register as a chaperone (for Entertainment Licences) - what do I need to do ? You should contact Attendance and Pupil Supportand we will arrange for a member of the team to contact you. They will need to visit you at home to ask you a few questions. |
